If none of them apply to you, click no and you can continue with the plan change form. Changes are effective on the first day of the month after you complete a plan change application. Upgrades to plans are only permitted during Open Enrollment .

Thats why BCBS FEP Dental offers the supplemental coverage you and your family need to keep your teeth healthy, including three free cleanings a year. Were proud to offer our coverage to federal employees and retirees plus eligible retired uniformed service members.
These benefits are neither offered nor guaranteed under contract with the FEHB Program but are made available to all enrollees and family members who become members of BCBS FEP Dental.

$1500 Annual Maximum Per Member Either Way You Choose
DentalPlus and DentalPlus PPO provide preventive, primary and major care for people of all ages at a monthly premium you can smile about.

* No copays or deductibles for exams, cleanings, x-rays and other preventive services outlined in the contract at in-network dentists up to the allowable charge and up to the annual $1,500 maximum for each member.
Exclusions: Any dental service not listed as a covered service in this program, patient education services, hospital calls and/or consultations, laboratory and pathological examinations, bone grafts for alveolar ridge augmentation, dental services primarily for cosmetic purposes, except for an accidental injury, all Temporomandibular Joint Dysfunction Syndrome related services, occlusal adjustments, temporary or provisional dental services and procedures, orthodontic services. This is a brief summary of the coverage available under this program. Please refer to the policy for a complete list of exclusions and limitations.

Get More From Your Health Plan
Its not too late to submit your Medicare application. However, the application must be received by Monday, December 7. That doesnt leave much time to drop it in the mail, so we have made it easy for you to sign up with BCBSRI.
You can complete an application online at bcbsri.com/medicare or call us at 351-2583 for assistance with your application.
We also will have BCBSRI representatives available at the following locations where you can drop off your enrollment form. A BCBSRI Medicare consultant will be available to review your application and make sure it is completed correctly.

How To Buy An Affordable Care Act Plan
You can purchase ACA healthcare plans through a federal marketplace called the Health Insurance Marketplace® or outside the Health Insurance Marketplace®. The route you should take depends on your individual healthcare needs and financial situation. Our representatives can answer your questions and help you navigate these choices. Available plans and pricing may vary depending on whether you purchase your plan on or off the Health Insurance Marketplace®.

Though most dentist offices will file a claim for you, if they don’t offer that service, you will need to file the claim.
Who’s eligible for Dental Blue® for Individuals?
All applicants and their dependents are eligible. Applicants must also be a resident of North Carolina and must have not had a Dental Blue for Individuals policy in the last 12 months.
How do I file a dental claim?
Participating providers will file the claim on your behalf. If your dentist office does not file claims, you should pay the dentist in full and submit your claim to Blue Cross NC for reimbursement. Complete a dental claim form and mail it to us within 180 days from the date of your service.

Will I get credit for my prior dental coverage?
Yes, Blue Cross NC may waive or reduce any applicable dental waiting period by the number of month of prior dental coverage. Proof of prior dental coverage with less than 63 days lapse in coverage is required.
Dental Blue for Individuals is not part of the covered health insurance benefits of any Blue Cross NC plans. Dental Blue for Individuals must be purchased separately.
