What Is Medicare Sequestration
Overall, Medicare sequestration reduces government spending to meet budgetary goals.
Essentially, sequestration reduces what Medicare pays its providers for health services by two percent.
However, Medicare beneficiaries bear no responsibility for the cost difference.
While aimed to prevent further debt, it imposes financially on hospitals, physicians, and other healthcare providers.
When it comes to Medicare sequestration, defining a sequester and how it applies to Medicare should come first.
According to the Congressional Research Service, sequestration is a reduction in federal spending by a certain percentage. As this applies to Medicare, the reduction in federal spending means providers receive less payment for services, specifically by two percent.
Important to note is that beneficiaries do not pay this extra two percent Medicare sequester.
Rather, the healthcare providers themselves bear the added financial burden.
Sequestration officially began in the US on April 1, 2013. However, Congress more recently suspended the Medicare sequester starting April of 2020 through December 31, 2021.

The reduction in payment to physicians also applies to hospitals, home health services, and durable medical equipment companies. Essentially, this includes any provider contracted with Medicare.
This applies to medical equipment rentals, prosthetics, and similar services. However, the decrease only applies if use of the item or service began on or after April 1, 2013.
Additionally, physician-administered drugs such as chemotherapy receive the 2 percent reduction.
What Is The Medicare Gap
The MBS lists a rebate amount for each Medicare Item.
Example: A short consultation with a Doctor = Medicare rebate of $15.35
However, doctors or specialists may charge more than the listed MBS amount.
The difference between what is charged and the MBS rebate is called the Medicare Gap.
- The Doctor charges $45.00.
- Medicare provides a rebate of $15.35.
- Medicare Gap = $29.65 .

Can A Person Have Both
A person can have both Medicare and private insurance at the same time. In these cases, Medicare establishes primary and secondary payers. The primary payer pays the claim first, while the secondary payer covers expenses that remain unfunded by the primary payer.
Medicare has various rules for establishing the primary payer. For example, Medicare is the primary payer when a person has private insurance through an employer with fewer than 20 employees. To determine their primary payer, a person should call their private insurer directly.
Comparison Of Administrative Costs
To help determine whether or not the federal governments Medicare program is more cost efficient than non-government health insurance, weve compared the administrative costs for both according to five different studies. The results are presented in summary chart form and graph form with links to the full studies themselves.
Some organizations, including the Heritage Foundation, argue that calculating administrative costs as a % of total expenses misleads people into thinking that Medicare is more efficient than non-government health insurance. They believe that comparing the administrative costs per beneficiary more accurately reflects the greater efficiency of private health insurance. Others point out that Medicare treats the elderly exclusively, and since they typically have more health problems than the general population, comparing costs per beneficiary is misleading and therefore percentage comparisons are more reliable.
An international comparison of the US and five other industrialized countries shows that the US spends at least twice as much administrating its health care system as countries with universal health coverage such as Germany and Canada.
Administrative costs, according to the Congressional Budget Office , refer to any expenses insurers incur that are not payments for health care services, including the profits retained by private insurers and the taxes paid on those profits.

How Medicare Sequestration Affects Physicians
As a medical fee-for-service program, Medicare pays its providers specific rates for specific services. However, payments to physicians reduce by 2 percent.
Under these budget cuts, any claim for medical services received by Medicare after April 1, 2013 got a 2 percent payment cut. Additionally, Medicare reimburses for any drugs administered as part of the claim with the same 2 percent cut.
The reduction in payment applies to the payment itself as opposed to modifying the allowed charge published by Medicare as part of their fee schedule.
Applied Behavior Analysis Medical Necessity Guide
The Applied Behavior Analysis Medical Necessity Guide helps determine appropriate levels and types of care for patients in need of evaluation and treatment for behavioral health conditions. The ABA Medical Necessity Guide does not constitute medical advice. Treating providers are solely responsible for medical advice and treatment of members. Members should discuss any matters related to their coverage or condition with their treating provider.
Each benefit plan defines which services are covered, which are excluded, and which are subject to dollar caps or other limits. Members and their providers will need to consult the member’s benefit plan to determine if there are any exclusions or other benefit limitations applicable to this service or supply.
The conclusion that a particular service or supply is medically necessary does not constitute a representation or warranty that this service or supply is covered for a particular member. The member’s benefit plan determines coverage. Some plans exclude coverage for services or supplies that Aetna considers medically necessary.

How Medicare Sequestration Affects Beneficiaries
Despite the effects on providers, the reduction in payments doesnt affect what patients pay for Medicare coverage. No changes in deductible amounts or cost-sharing occurred.
Beneficiaries themselves have no responsibility for paying additional costs due to the Medicare sequester.
Some analyst believe Medicare sequestration has caused a trickledown effect in healthcare.
Why You Should Be Par With Medicare
- Your Medicare fee schedule amount is 5% higher than that of a non-participating provider.
- Collections from patients are much easier because Medicare reimburses 80% of the allowed charges to the provider and the practice will have to collect the remaining 20% from the beneficiary.
- Medicare will automatically forward Medigap claims to the proper insurer for payment when they receive the completed claim form. This one-stop billing eliminates the need to submit a separate bill to the supplementary insurer or beneficiary after receiving Medicares payment.
- Participation also improves the relationship with the beneficiary as it helps reduce any out-of-pocket expenses that will be the responsibility of the beneficiary.

Private Health Insurance Vs Medicare: Who Pays For What
How Medicare works, and what private health insurance can do for you.
- Private health cover is divided into three areas: hospital, extras and ambulance
- Hospital cover enables you to choose your doctor and hospital
- Extras cover can help with the cost of services not covered by Medicare
Medicare is available to Australian citizens and most permanent residents. It covers part of the cost of seeing a GP or specialist , and most prescription medicines. It also provides free treatment and accommodation in public hospitals.
As a public patient in a public hospital youll be treated by hospital-appointed doctors and subject to waitlists for non-emergency treatment.
Medicare doesnt cover the cost of ambulances, glasses/contact lenses or hearing aids. It also excludes therapies such as speech pathology, osteopathy and remedial massage.
Private health insurance can fill the gaps in Medicares coverage and give you more choice about your treatment.

Is The Hassle Of Dealing With Medicare Paperwork And Regulations Really Worth The Extra 15% You Can Bill As A Non
Dealing with Medicare is complex and riddled with headaches, but there are a number of other things to consider when asking yourself this question. First and foremost, you need to put the 15% topic aside and look at how important it is for your practice to be able to provide covered PT services to Medicare beneficiaries. Are you in an area where the vast majority of those seeking PT are over 64 years old? Could you keep your practice busy if you were not able to treat this part of the population? Remember, if you are not enrolled as either a Participating or a Non-Participating provider, you cannot provide beneficiaries with PT services that Medicare would normally cover.
You should also consider how important it is to you as a practitioner to be able to treat Medicare beneficiaries. I know some PTs who are not really concerned about having or not having that ability in their practice, and I know some who couldnt imagine practicing PT if they were not allowed to treat Medicare-aged patients.
If you feel that you need to be able to treat Medicare beneficiaries, either financially or personally, but dont want to wait for Medicare reimbursement , then being a Non-Par Provider might be a an option to consider regardless of the extra 15% you can bill.
